Remote monitoring of hypertension medication adherence

Problem
Many hypertensive patients struggle to take their medication consistently due to forgetfulness, complex routines, or living alone. This leads to missed or incorrect doses, increasing health risks and making it difficult for doctors to monitor patients’ adherence remotely and provide timely interventions.
Solution
We developed a remote medication tracking system that combines a frontend UI with a backend infrastructure to monitor adherence. The system detects pill removal and user gestures using LDR sensors and a camera, while transmitting real-time data through Firebase. This bridges the gap between patients and healthcare providers.
Understanding Patient Barriers

Globally, hypertension affects over 1 billion people, yet poor medication adherence 30-40% remains a major barrier to effective treatment, leading to serious health risks and increased healthcare costs.
The most prevalent reasons include: forgetfulness, failure to perceive the need for medication, fear of side effects and low self-efficacy.
There is a clear need for an integrated hardware and software solution that leverages T-Mobile’s connectivity to enable real-time tracking, reminders, and data sharing to empower patients, and reduce caregiver burden and improving health outcomes.
Insights from User Research
I interviewed four patients with hypertension from different age groups to know how they struggle in their daily routines.


Design Strategies
Based on the analysis above, we formed the goals to help hypertensive patients accurately take medicines through the two-way smart tracking system that automatically syncs data to doctor’s side. We are building a two-part system to track medication adherence and support hypertension care:
